Normal Vaginal pH
The vaginal environment is naturally acidic, with a physiologic pH typically ranging from 3.8 to 4.5. This acidic pH plays a key role in maintaining a healthy microbiota and inhibiting the growth of pathogenic bacteria and fungi.
Clinical Indications for Alkaline (Basic) Vaginal Washes
Alkaline washes, characterized by a pH above 7, are considered therapeutic agents and are generally used under medical supervision for specific conditions, including:
* Adjunctive management of vulvovaginal candidiasis: Certain fungal infections may benefit from an alkaline environment to help reduce fungal viability.
* Treatment of specific vaginal infections or abnormal discharge: May be recommended as part of a targeted treatment protocol.
* Use in gender-selection methods (theoretical): Some hypotheses suggest that an alkaline vaginal environment may favor Y-chromosome sperm; however, this is not evidence-based and may disrupt normal vaginal physiology.
Daily Use Recommendation:
pH-balanced acidic cleansers (3.8–4.5) are the safest option for everyday external hygiene to maintain a healthy vaginal microbiome.
Medical Use Only:
Alkaline washes should be used only when medically indicated and for a limited duration, as advised by a healthcare provider.
